Objectives: The occurrence of erectile dysfunction (ED) due to anti-hypertensive drugs has been frequently reported, and many studies suggested the causality of β-blockers in increasing the ED risk. However, unlike other β-blockers, nebivolol is known to reduce the risk of ED occurrence or to improve erectile function. In this study, we conducted a systematic review and meta-analysis to compare the ED incidence between nebivolol and other β-blockers. Methods: The search for literature was conducted on May 19?21, 2020 through PubMed, Embase, and Cochrane. Using a random- effects model, the odds ratio for the occurrence of ED was computed for studies included for meta-analysis. The mean difference of the International Index of Erectile Function (IIEF) scores before and after drug treatments was also compared between patients treated with Nebivolol and with other β-blockers. Results: The probability of ED occurrence in the nebivolol group was 0.69 times higher than that in other β-blockers group [95% confidence interval (CI): 0.31?1.53, heterogeneity: I2=0%]. In addition, the post-drug IIEF scores of the nebivolol group were 1.07 points less reduced than that of other β-blockers groups (95% CI: 0.69?1.45, heterogeneity: I2=0%). Conclusion: Our results showed that the IIEF score had a smaller drop in nebivolol group than that in other β-blockers groups. Although the difference of ED incidence between two groups was not significant, all studies showed same results that the ED incidence is lower in nebivolol group than in other β-blockers group. Thus, we confirm that, unlike other β-blockers, nebivolol has a clinical effect that protects the sexual function of men.
