Current models of ADHD are based on neuropsychological theories of impaired functioning of the frontal lobes, particularly the prefrontal cortex (PFC). Children with ADHD show improvements in performance on tests of executive function, attention, and vigilance following stimulant treatment. The aim of this study was to evaluate the effects of methylphenidate-OROS (MPH-OROS) on cognitive function and intellectual performance in Korean children with ADHD. Ninety-one elementary school children with ADHD (combined type), 6-12 years old, participated in the multicenter, 4-week open trial of MPH-OROS. Function was assessed using behavioral checklists, the KWISC-III, Continuous Performance Test (CPT), Trail-Making Test (TMT), and Wisconsin Card Sorting Test (WCST). In total, 83 subjects were analyzed using paired t-tests (repeated measures). After MPH-OROS treatment, significant improvements (p<0.01) were observed in the WISC-III, with reduced auditory omission errors, commission errors, and visual omission errors on the CPT. TMT and perseverative response times improved and perseverative errors decreased significantly (p<0.01); perseverative errors and categories completed on the WCST improved significantly. The majority of patients (79.5%) showed at least one adverse effect, with most being mild. Common adverse effects were appetite loss (19.2%), insomnia (19.2%), and headache (9.6%). Although our findings suggest that MPH-OROS has beneficial effects on cognitive and intellectual function, especially on executive function, in children with ADHD, a further long-term follow-up study of these effects is warranted.
