배경 : Metallo- -lactamase (MBL)은 국내에서 Pseudomonasspp. 및 Acinetobacter spp.에서 검출 빈도가 매우높아지고 있으며, Serratia marcescens, Enterobacter cloacae 및 Klebsiellapneumoniae 등의 장내세균에서도 VIM-2형 MBL이 분리된 바있다. 최근에 저자들은 신경외과 병실에 있는 3명의 환자 소변에서분리된 3주의 MBL 생성Providencia rettgeri의 표현형과 유전형을 분석하고 한 환자에서 분리된 Providencia rettgeriCBU852균주의 blaVIM-2를 포함하는 integron을 분석하였다.방법 : EDTA-disk synergy test로 MBL을 선별하였으며,IMP-1, VIM-1, VIM-2 유전자에 대한 PCR을 시행하였다. 최소억제농도는 미량액체배지희석법을 이용하였고, random ampli-fied polymorphic DNA를 이용하여 DNA fingerprint를 시행하였다. blaVIM-2를 포함하는 integron의 염기서열을 분석하였다.
결과 : Imipenem 내성 P. rettgeri3주 모두 EDTA-disk syn-ergy test에서 양성이었고 PCR 결과 VIM-2형 MBL이 검출되었다. 항균제 감수성 검사에서 시험한 모든 -lactam제에 내성일뿐만 아니라 gentamicin, tobramycin, amikacin 및 ciprofloxacin에도 내성을 나타내었다. P. rettgeri CBU852는 aacA4, blaVIM-2,orf‘‘ii", orf‘‘iii"를 포함하는 새로운 구조의 class 1 integron을갖고 있었다. 이들 균주의 RAPD 양상은 동일하였다.
결론 : 이 연구에서 다제내성인 P. rettgeri CBU852는 blaVIM-2를 포함하는 새로운 class 1 integron을 갖고 있었다. P. rettgeri에 까지 blaVIM-2의 확산은 이들 균에 의한 감염의 치료에서 carbapenem을 무력화할 수 있으며, 이 보고는 VIM-2형 MBL을 생성하는 P. rettgeri의 첫 예이다.
