배경 : i-STAT은 신속한 결과를 제공하며 검체요구량이 적어특히 신생아 진료에 있어 긍정적인 효과를 가져올 수 있다. 그러나, 신생아중환자실(neonatal intensive care unit, NICU)에서i-STAT을 일상적으로 사용할 것인지의 여부를 결정하기 위해서는 기기의 분석능 뿐 아니라 비용효율적인 측면에 대한 고려가 있어야 한다. 본 연구자들은 NICU의 i-STAT 도입에 있어 임상적인 측면에 대한 평가를 시도하고자 하였다. 방법 :본원에서의 전반적인 현장검사 이용현황을 조사하기 위하여 의사 및 간호사들을 대상으로 설문조사를 시행하였다. NICU입원 중인 신생아들을 대상으로 중앙검사실에서 동맥혈가스분석기로 검사를 시행한 84예, NICU에서 기존의 동맥혈가스분석기로 검사를 시행한 88예, i-STAT으로 검사한 96예에 대하여 분석하였다. 각 사례별로 검사의 적응증, 채혈부터 결과보고까지의 소요시간(turnaround time, TAT), 검사의뢰시점부터 진료에 반영하는시점까지의 소요시간(cycle time), 치료에 대한 영향 등을 분석하였다. 또한 검사 비용과 사용자 만족도를 조사하였다.결과 :설문응답자들은 공통적으로 신속한 검사결과보고를 원하였으나, 반면 현장검사의 정도관리에 대한 책임의식이 부족하였다.i-STAT은 중앙검사실에서 또는 NICU의 동맥혈가스분석기를 이용한 검사에 비해 TAT를 현저히 단축시켰으나, cy cle time 단축에는 큰 영향을 주지 못하였다. i-STAT을 이용한 동맥혈가스분석시의 검사비용은 중앙검사실 검사의 2.2배 정도 고가였다. 그러나,사용자들은 검체요구량이나 신속성, 이동성 등에 있어 i-STAT에더 높은 만족도를 보였다. i-STAT 도입 후 중앙검사실로 의뢰되는 검사건수는 14.3% 감소되었다.
결론 : i-STAT은 NICU 환경에 적합한 현장검사기기로 알려져있으나, 비용효율성을 극대화하기 위해서는 선택적인 이용 및 임상의의 태도 변화가 반드시 전제되어야만 할 것이다. 또한 i-STAT의 임상적 유용성을 증명하기 위해서는 추후 연구가 더 필요할 것으로 사료된다.
