Objective: Alzheimer’s disease (AD) is accompanied by cognitive impairment, which significantly affects patients’ quality of life. The purpose of this study is to systematically review the health-related quality of life (HRQoL) of patients with AD as measured by the EQ-5D. Methods: We used eight databases: Ovid MEDLINE, EMBASE, The Cochrane Library, KMBASE, KoreaMed, KISS, ScienceOn, and RISS. We searched until August 2023 using “AD” and “EQ-5D” as search terms. After selecting articles that met the inclusion criteria, we retrieved articles from 1999 to 2021 and included one meta-analysis. Results: The search yielded eleven articles, two of which focused on the Korean population. Seven of the articles used the Mini Mental State Examination to measure patients’ disease severity, three used the Clinical Dementia Rating, and one used both. All studies used proxy EQ-5D questionnaires, with five studies presenting patient self-reported and caregiver-rated quality of life separately. The meta-analysis showed that the mean EQ-5D scores for mild cognitive impairment, mild, moderate, and severe AD were 0.76, 0.68, 0.54, and 0.32 for caregiver-rated assessments, and 0.74, 0.67, 0.61, and 0.32 for self-reported assessments. Conclusion: This systematic review shows that AD patients’ quality of life declines as the severity of AD increases. This finding helps us understand the societal and individual burden of AD. Further research is needed to explore the factors that influence quality of life and its measurement in people with AD.
