본 연구의 목적은 아동?청소년기 대인외상경험이 있는 폭식장애경향 성인 여성을 위한 이미지 재구성 인지행동치료 프로그램을 개발, 실시한 후 전통적인 인지행동치료 프로그램과 그 효과를 비교하는 것이다. 연구대상은 대인외상경험 체크리스트 5점 이상, BULIT-R 88점 이상의 중복 기준에 해당되는 20∼30대 여성을 1차 선별한 후 2차적으로 심상 면접을 실시하여 최종 선별된 성인 여성 21명을 이미지 재구성 인지행동치료를 실시한 실험집단, 전통적인 인지행동치료를 실시한 비교집단 및 무처치 통제집단에 각각 7명씩 배정하였다. 프로그램의 효과는 사전, 사후 및 3개월 추후 검사를 통해 검증하였다. 본 연구의 주요 결과는 다음과 같다. 첫째, 프로그램 실시 후 섭식 변인인 부정적 정서로 유발된 섭식 동기?행동은 실험집단이 통제집단에 비해 낮은 것으로 나타났으며, 인지 변인인 역기능적 신념, 부정적 자동적 사고, 자아존중감 중 역기능적 신념은 실험집단이 비교집단과 통제집단에 비해 낮았고, 부정적 자동적 사고는 실험집단과 비교집단이 통제집단에 비해 낮았으며 자아존중감은 실험집단이 통제집단에 비해 높은 것으로 나타났다. 둘째, 프로그램 실시 후 정서 변인인 정서조절곤란, 우울, 상태-특성 불안, 상태-특성 분노 중 우울은 실험집단이 비교집단과 통제집단에 비해 낮았고, 정서조절곤란, 상태-특성 불안, 상태-특성 분노는 실험집단이 통제집단에 비해 낮은 것으로 나타났다. 셋째, 프로그램 실시 후 행동 변인인 스트레스 대처방식의 문제중심적 대처와 사회적지지 추구는 실험집단이 통제집단에 비해 높았으며, 정서완화적 대처는 실험집단이 통제집단에 비해 낮은 것으로 나타났다. 넷째, 프로그램 종결 3개월 후 섭식(부정적 정서로 유발된 섭식 동기?행동), 인지(역기능적 신념, 부정적 자동적 사고), 정서(정서조절곤란, 우울, 상태-특성 불안, 상태-특성 분노), 행동(스트레스 대처방식 중 정서완화적 대처) 변인은 실험집단이 비교집단에 비해 낮은 것으로 나타났다. 반면, 인지(자아존중감), 행동(스트레스 대처방식의 문제중심적 대처, 사회적지지 추구) 변인은 실험집단이 비교집단에 비해 높은 것으로 나타났다.
The purpose of this study is to develop and implement a imagery rescripting cognitive-behavioral therapy program for women with binge eating tendencies, who have had a traumatic experience in interpersonal relationships during their childhood/adolescence and to compare its effectiveness with that of the traditional cognitive-behavioral therapy program. As research subjects, firstly, women in their 20s and 30s were selected, who met the criteria of five points or higher in the checklist for traumatic experience in interpersonal relationships and 88 points or higher in the BULIT-R. Secondly, imagery interviews were conducted with them, and 21 women were finally selected, and seven women were assigned, respectively, to an experimental group that received imagery rescripting cognitive-behavioral therapy, a comparison group that received the traditional cognitive-behavioral therapy was conducted and an untreated control group. The effectiveness of the program was validated through pre-testing, post-testing, and a three-month follow-up. The main findings of this study are as follows: First, after the program was implemented, the food intake motive/behavior induced by negative emotion, a food intake variable, was lower in the experimental group than in the control group, and of the cognition variables, such as dysfunctional beliefs, negative automatic thoughts and self-esteem, dysfunctional beliefs were lower in the experimental group than in the comparison group and the control group. The experimental group and the comparison group had fewer negative automatic thoughts than the control group did. Self-esteem was higher in the experimental group than in the control group. Second, after the program was implemented, the depression one of emotional variables, such as difficulties in emotion regulation, depression, state-trait anxiety and state-trait anger were lower in the experimental group than in the comparison group and control group. And the difficulties in emotion regulation, state-trait anxiety and state-trait anger were lower in the experimental group than in the control group. Third, after the program was implemented, problem-focused coping of stress-coping style which is the behavioral variables and social support pursuit were higher in the experimental group than in the control group, while emotion-alleviation coping were lower in the experimental group than in the control group. Fourth, three months after the program was finished, food intake(food intake motive/behavior induced by negative emotions), cognition(dysfunctional beliefs, negative automatic thoughts), emotion(difficulties in emotion regulation, depression, state-trait anxiety and state-trait anger), and behavior(emotional palliative coping of stress-coping style) variable were lower in the experimental group than in the comparison group. On the other side, cognition(self-esteem) and behavior(problem-focused coping of stress-coping style, pursuing social support) variable were higher in the experimental group than in the comparison group.
