메트로놈 요법은 독성이 나타나지 않는 낮은 용량으로 항암제를 지속적으로 빈번하게 투여하는 방법으로, 기존의 항암화학요법과 비교해 보았을 때 독성이 적고, 내성이 발생할 가능성이 낮다는 장점이 있다. 동물실험에서 간세포암에 대해 우수한 항신생혈관작용과 항암 효과를 보여 의미있게 생존기간이 연장되었음을 보고되었으나, 임상적으로는 간세포암 치료에서 메트로놈 요법을 적용한 예는 보고된 바 없었다. 본 증례는 각각 폐전이와 주문맥 혈전을 동반한 진행성 간세포암 환자에서 대퇴동맥을 통해 간동맥에 삽관하여 반영구적 포트를 장착한 후 이를 이용한 메트로놈 항암요법을 통하여 완전관해의 뛰어난 치료 효과를 경험한 첫번째 예로, 문헌고찰과 함께 보고하는 바이다.
