Objective : The purpose of this study was to examine the regional cerebral blood-flow differences between children with pure ADHD and normal controls, and associations between blood-flow abnormalities and cognitive characteristics in children with ADHD.
Methods : The cerebral blood-flow of fourteen children with pure ADHD and ten normal controls were examined by Tc-99m ECD SPECT and cognitive characteristics in children with pure ADHD were measured by ADS. First, regional blood-flow differences between two groups were analyzed by using SPM. Second, associations between types of cognitive characteristics measured by ADS and cerebral regions showing significant difference were examined, and quantitative associations between severity of cognitive impairments and degree of blood-flow decrease were analyzed by using Pearson's coefficient correlation.
Results : 1) The cerebral blood-flows of children with ADHD were significantly decreased in left orbitofrontal area and left superior temporal gyrus compared to normal controls. 2) There were tendency of positive correlation between the degree of commission error(impulsivity, disinhibition) among the ADS variables and the degree of blood-flow decrease above two areas.
Conclusion : These results suggest that the pathophysiology of impulsivity or disinhibition in the characteristic symptoms of ADHD is associated with hypoactivities of left orbitofrontal area and left superior temporal gyrus.
