Sertraline is a potent selective inhibitor of central serotonin reuptake. It enhances serotonergic transmission which appears to explain its antidepressant activity. Although sertraline is not different from TCAs in efficacy and onset of action in the treatment of depression, it has minimal anticholinergic, sedative activity, lack of cardiovascular effect, has a wide therapeutic index, and no significant effect on psychomotor and cognitive performance, thus may be administered to elderly patients.
Sertraline has no direct effect on central noradrenergic and dopaminergic systems, but does cause down-regulation of central beta-adreonceptors.
The recommended starting dosage of sertraline for the treatment and prophylaxis of depression is 50㎎ once daily, its elimination half life makes it suitable for once daily administration.
Sertraline 50-200㎎/day has been very well tolerated. Adverse events are usually dose-related, mild and transient, and decrease in frequency as treatment is continued.
