Introduction: Hemifacial hypertrophy (HFH) is a congenital morphological disorder that causes facial asymmetry resulting from unilateral overgrowth of tissues on the affected side, including soft tissues, bone, and teeth. HFH occasionally includes an enlarged tongue and lower lip on the affected side. The etiology of HFH is poorly understood and multiple theories have been postulated. Case report: This case report presents and evaluates the uncommon dentofacial characteristics, including accelerated tooth eruption, regional macrodontia, and HFH of the hard and soft tissues and muscles biased to the right side, in a young patient with HFH. It is noteworthy that the right HFH was accompanied by facial infiltrating lipomatosis on the ipsilateral side and the histological examination was needed for further diagnosis. Based on these findings, the patient was advised to receive long-term dental care, including orthodontic management of the developing malocclusion. Conclusion: As clinical and radiographic examinations are important element in the diagnosis of HFH and facial infiltrating lipomatosis, this case report will be useful for the diagnosis of similar cases in the future.
