Purpose
Critically ill surgical patients in the intensive care unit (ICU) show rapid decrease in the amount of skeletal muscle mass (SMM). This skeletal muscle wasting is often masked by fluid retention and edema - skeletal muscle mass from bioelectrical impedance analysis (BIA) can be over-measured or inaccurate in edematous critically ill patients. Thus, extracellular water (ECW) ratio should be taken into consideration when assessing sarcopenia of ICU patients. The aim of this study is to analyze the clinical outcome of surgical ICU patients according to skeletal muscle mass index (SMI) and edematous status (ECW ratio, extracellular water/total body water; ECW/TBW) from BIA.
Methods
A total of 406 critically ill surgical patients over 18 years old (male 256, female 150, mean age 60.8 years old; range 18-93) admitted in Korea University Anam Hospital was enrolled in this study. BIA was measured twice a week (Monday and Thursday) by a researcher. Because SMI is different according to gender, male and female patients were analyzed separately.
