Purpose
Risk factors of pneumatosis intestinalis (PI) related to mortality have been developed, and algorithms and scoring systems that help clinical judgment have been suggested, but there is no standardized protocol so far. Therefore, we aim to create a new score system to determine the prognosis of PI used in clinical practice.

Methods
From January 2006 to January 2021, we enrolled the patients who were diagnosed with PI. We excluded children, diagnosis without abdominopelvic computed tomography, omission of medical records, explorative surgery, pneumatosis except for intestinalis, transfer to another hospital, and treatment for multiple diagnoses other than PI. A total of 120 patients were analyzed retrospectively. Eighty-seven patients were treated non-surgically, and thirty-three patients underwent surgery. Univariate and multivariate logistic regression analyses were used to identify risk factors for prognosis of PI. Risk factors for mortality include white blood cell count, neutrophil lymphocyte ratio (NLR), pH, HCO3, lactate, total bilirubin, creatinine, INR, systolic blood pressure (SBP), heart rate, body temperature, PI on small bowel, and portal vein gas. Cut-off values for prognosis were assessed by using the receiver operator characteristic (ROC) curve.
