Background
The current eighth edition of the American Joint Committee on Cancer staging manual on colon cancer included the tumor deposits (TDs) only for node-negative patients, as N1c, and ignored the counts of which. This study evaluated the prognostic value of TD counts and established a revised nodal staging system.
Methods
We conducted a retrospective review of medical records of patients with stage III colon cancer underwent curative resection in our institution between December 2009 and December 2019. The patients were grouped based on the number of TD counts: TD 0, TD 1-2, and TD ≥ 3. Univariable and multivariable regression analysis was performed to determine the independent prognostic factor for disease-free survival and overall survival. With incorporating TD counts into positive lymph nodes, patients were restaged and additional nodal stage, 'N2c', was defined as those with ten or more integrated lymph nodes. The predictability of revised nodal staging system was compared with current AJCC 8th nodal staging system in terms of concordance indices. Kaplan-Meier curve analysis and log-rank test were performed to compare disease-free survival and overall survival.
