배경 : Methicillin 내성 Staphylococcus aureus (MRSA)와296 이종한김창기노경호 외 5인일부 그람음성 간균은 흔한 원내 감염균이고 여러 항균제에 내성인 균주가 많으며, 흔히 중복 감염을 일으킨다. Arbekacin은 ami-noglycoside제로서 MRSA와 일부 그람음성 간균에 대한 시험관내 항균력이 우수한 것으로 알려져 있다. 본 연구에서는Staphy-lococcus균종과 그람음성 간균에 대한 arbekacin의 시험관내 항균력을 시험하고자 하였다. 재 료 및 방 법 : 세브란스병원 환자의 임상검체에서 2003년에분리된 균주를 대상으로 하였다. 항균제 감수성은 CLSI 한천희석법으로 시험하였고, arbekacin의 breakpoint는 ≤4 g/mL를감수성, 8 g/mL를 중간, ≥16 g/mL를 내성으로 하였다.결과 : 모든 Staphylococcus 균주는 methicillin 내성 여부에관계없이 arbekacin ≤4 g/mL에서 억제되었고, 내성인 균주는없었다. Arbekacin의 MIC 90은 1-4 g/mL로, amikacin과 gen-tamicin의 MIC90보다 각각 8->32배와 >32-128배 낮았다. Methi-cillin 감수성 Staphylococcus aureus, Methicillin 내성 coagulase-negative staphylococci 및 Methicillin 감수성 coagulase-nega-tive staphylococci의내성률은 arbekacin에 대해 0%, amikacin 에0-54%, gentamicin에 24-79%이었다. Escherichia coli와 Citro-bacter freundii에 대한 arbekacin의 MIC90은 각 1 g/mL와16 g/mL이었고, amikacin과 gentamicin의 MIC 90보다 각각2-4배와 8-16배 낮았다. 그러나 다른 그람음성 간균에 대한 arbe-kacin의 MIC 90은 64->128 g/mL로 다른 aminoglycoside 항균제와 비슷하였다.
