괴사성 근막염은 급속히 진행하여 생명을 위협할 수 있는심각한 연부조직 감염이다. 기저 질환이 있는 경우 그람 음성균에 의해 잘 발생하는데, 원인 균주로 E. coli가 배양되는경우는 대부분 다균주 혼합 감염이었으며 단독 균주로 보고되는 경우는 드물고, 지역사회에서 발생한 ESBL 생성 E. coli 에 의해 괴사성 근막염이 발생한 보고는 없었다. 최근 ESBL 생성 E. coli는 전 세계적으로 중요한 지역사회 획득 균주로대두되고 있다. 이에 저자들은 간경화 환자에서, 지역사회에서 발생한 ESBL 생성 E. coli에 의한 단독 감염으로 괴사성근막염이 발생한 사례를 경험하였기에 문헌고찰과 함께 보고하고자 한다.
