Background: Leptin is a peptide hormone secreted by adipocyte and its function is the regulation of body weight through reducing food intake and stimulating energy expenditures. Leptin receptor (LEPR) is located at the brain, liver, pancreas and muscles. Mutations of the LEPR gene cause inactivation of LERP, then hyperleptinemia and obesity appear. It is known that hyperleptinemia involves in the pathogenesis of ischemic stroke such as atherosclerotic process, endothelial dysfunction, and platelet aggregation. To investigate the effect of LERP gene to cerebral infarction, we studied the LEPR Gln223Arg polymorphism and plasma leptin levels in patients with ischemic stroke. Methods: 64 patients (36 men, 28 women) with ischemic stroke and 53 healthy individuals (28 men, 25 women) were enrolled in this study. We measured their fasting body mass index (BMI), blood sugar (FBS), total cholesterol, triglyceride (TG), high-density lipoprotein (HDL), low-density lipoprotein (LDL), and leptin levels. The LEPR Gln223Arg polymorphism was analyzed using standard PCR-RFLP method. Results:The Gln/Gln, Gln/Arg, and Arg/Arg genotypes were 32.8%, 35.9%, 31.3% in patients and 52.8%, 24.5%, 22.6% in control, respectively. Plasma leptin levels were significantly higher in patients (8.39 ± 5.26 μg/L) than control (5.64 ± 4.09 μg/L) (p = 0.003). Plasma leptin levels were correlated with BMI and significantly higher in female and the Arg/Arg than Gln/Gln and Gln/Arg genotype in patients. Adjusted odd ratio and 95% confidence intervals were 1.09 (1.04 to 1.15) for the presence of Arg allele. Conclusions: Our findings suggest that leptin and Gln223Arg polymorphism are independent risk factors for ischemic stroke.
