Background：Sudden hearing difficulty is a rare manifestation of vertebrobasilar occlusive disease.
Case：A 62-year-old man was admitted because of bilateral hearing difficulty and dizziness. Brain magnetic resonance imaging revealed acute paramedian and left lateral pontine infarction. Brain magnetic resonance angiography showed severe stenosis of the both distal vertebral arteries and mild stenosis of the distal basilar artery. Pure tone audiometry showed moderate to severe bilateral sensorineural hearing loss. Brainstem auditory evoked potentials revealed bilateral poor formation of the wave I and II, which is suggestive of cochlear nerve dysfunction.
Conclusion：Vertebrobasilar occlusive disease can cause bilateral cochlear nerve damage presenting with sudden bilateral hearing loss. (Korean Journal of Stroke 2006;8:213-216)
