This study was to investigate the changes of homocysteine and cognition according to exercise type in stroke patients. This study was conducted on 47 stroke patients. They were randomly divided into the aerobic exercise group(AE, n=18), resistance exercise group(RE, n=16), and control group(CON, n=13). We examined homocysteine, cognition, and stroke risk factors before and after 12 weeks of exercise training. 50 minutes exercise were performed three times with 70% HRmax(heart rate maximum) in AE and 65~75% 1RM(repetition maximum) in RE during 12 weeks. Body weight was reduced in both groups after 12 week exercise training(p<.01). Triglyceride(p<.05) and homocysteine(p<.05) were decreased and high-lipoprotein cholesterol(p<.01) was increased in both groups after 12 week exercise training. Cognition(p<.01) was increased in both groups after 12 week exercise training. The changes of homocysteine has shown a positive association with the changes of cognition(p<.05). However, there was no significant differences according to exercise type. These results suggested that exercise training had positive effects on changes in body composition, homocysteine, cognition, and stroke risk factors in stroke patients, but there was no difference according to exercise type.
