Background : Patients who have asthma or Chronic Obstructive Pulmonary Disease (COPD) mainly use inhalers for treatment. Correct use of inhalers is crucial for treatment. At Seoul St. Mary’s Hospital, pharmacists provide face-to-face consultations to patients who use their first inhalers, however, while pharmacists can easily assess their understanding of inhaler use immediately after consultation, it has been a challenge to check their proficiency and adherence after consultation. Therefore, we offered additional two non-face-to-face consultations (offering a video guide and, a 2nd telephone consultation). Through evaluating patient adherence, accuracy, understanding of inhaler use, and consultation satisfaction, we suggest improved inhaler consultation methods. Methods : Participants were 19 to 75 years old and used their first inhalers for asthma or COPD. They were hospitalized or treated in outpatient settings from September 2020 to August 2021. The study was conducted by sending a video guides in 2 weeks after 1st consultation. 2nd consultation was conducted 4 weeks after 1st consultation. Accuracy of inhaler use, understanding of inhaler use, and consultation satisfaction were evaluated in 1st consultation Adherence was evaluated in 2 weeks after 1st consultation. Other outcomes were evaluated in 4 weeks after 1st consultation. Results : Among 24 patients, 13 patients completed 2nd consultation. Average adherence score increased from 86.54% to 88.46% (P=0.673). There were no significant changes in the accuracy of inhaler use (P=0.680). The average level of understanding of inhaler use decreased from 2.92 to 2.17 points (P=0.095). Consultation satisfaction decreased from 11.08 to 10.00 points (P=0.100). Conclusion : Although there were no significant differences in any outcome, an increase in adherence was shown. Even though consultation satisfaction decreased, there was a positive response. Therefore, additional non-face-to-face consultations are expected to positively influence patients’ inhaler use. Because the number of study subjects was limited due to COVID-19, larger-scale research is needed to develop consultation methods.
