불면증으로 의뢰된 입원환자의 임상적 특징 및 협진 유형 분석

목 적:본연구의목적은입원환자들중불면증으로정신건강의학과로 자문을 요청하게 되는 환자들의 인구학적 특성과 협진을 의뢰하는 형태를 알아보고, 또한 비 재협진 군과 재협진군의 인구학적 특성과 협진 유형의 차이를 살 펴봄으로써, 종합병원 입원 환자의 불면증의 특징을 조사하 는 것이다. 방 법:2005년8월1일부터2011년12월31일까지약6년 6개월 간 입원한 환자들 중 정신건강의학과로 자문 의뢰된 4,966명의 환자를 대상으로 하였고 이중 불면증으로 의뢰된 236명의 환자들의 협진 결과를 이용하여 후향적으로 인구 학적특성및협진유형에대해분석하였다.또한,전체대 상자를 재협진군과 비 재협진군으로 구분하여 두 군의 인구 학적 특성과 협진 유형의 차이에 대해 분석하고자 하였다. 결 과:6년6개월간,병원에입원한전체환자중정신 건강의학과에 불면증으로 의뢰된 환자는 총 236명으로 전 체 정신건강의학과 협진 중 4.8%에 해당하였다. 불면증으로 의뢰된 환자의 평균 연령은 62.0 ± 14.3세로 전체 정신건강 의학과에협진의뢰된환자군의평균56.7±18.7세보다 높았다. 이중 재협진 의뢰 되었던 환자는 65명으로 전체의 27.5%를 차지 하였고 이는 전체 정신건강의학과 협진 의뢰 환자 중 평균 재협진 환자 비율(41.5%)에 비해 낮았다. 불면 증으로의뢰된환자군의협진의뢰형태는3형의뢰형태 가(입원 후 생긴 정신건강의학과적 합병증 치료를 위한 협 진 의뢰) 가장 많았고(51.3%) 2형 의뢰(입원 상의 문제와 정 신건강의학과적 증상과의 직접적인 관계가 없는 경우)가 2 번째로 많았다(36.0%). 결 론:대학 병원에서 불면증으로 의뢰된 환자 군은 전 체정신건강의학과협진의뢰환자군에비해고령,남성의 비율이 높았고, 재협진률이 낮으며 2차적으로 발생한 경우 가많았다.각과주치의들은입원환자들의불면증에발생 가능성에 유의하고, 필요시 적극적인 평가와 자문 의뢰를 시행하여야 하며, 자문 의뢰를 받은 정신건강의학과 의사들 은초진시진단과치료와함께재협진시기권고와같은적 극적인 개입을 할 필요가 있다.

Objectives: The purpose of this study was to explore insomniac demographic characteristics and the type of consultation provided to hospitalized patients asked to the Department of Psychiatry for insomnia and to compare patient insomnia charac- teristics by consultation type. Methods: We performed a retrospective chart review of 4,966 patients who were hospitalized from August 1, 2005 to Decem- ber 31, 2011 that received consultation in the Department of Psychiatry. Among them, 236 patients were referred for insomnia. We compared the differences in demographic characteristics and types of consultation between the insomnia patient group and other patient group. We also compared the difference between demographic characteristics and type of consultation by dividing total subjects into ‘with reconsultation’ and ‘without reconsultation’ groups. Results: Our results came from the analysis of 9,689 consecutive consultation requests. There were 4,966 patients that partici- pated in the study over 6 years and 6 months. The overall consultation rate was 3.3% of all admissions and insomnia patients comprised 4.8% of those. The ratio of re-consultation for insomnia was 27.5%. There was no signi cant difference in mean age between the insomnia ‘with reconsultation group’ and the insomnia ‘without reconsultation group’, but the ‘with reconsultation’ group had signi cantly more male patients and medical patients than the ‘without re-consultation’ group. For insomnia patients, consultation types were in the order of Mending request (51.3%), Paralle request (36.6%), Complementary request (9.0%) and this composition differed from that of total admission patients. Conclusion: Hospitalized patients referred for insomnia showed a higher proportion of male patients, lower rates of re-consul- tation compared with other patients, and most of these were for secondary insomnia. Each doctor should be aware of the possibility of inpatient insomnia, conduct positive assessments and referrals as necessary, and psychiatrists who might be asked for consultation need to prepare an active intervention with initial diagnosis and treatment, as well as recommendations for the timing of reconsultation.
