Kaposi’s varicelliform eruption (KVE) is a disseminated cutaneous infection caused by several viruses such as a herpes simplex virus (HSV) type 1 and type 2, coxsackievirus A16, vaccinia virus in patients with an underlying dermatosis. KVE is most often seen in patients with underlying atopic dermatitis but has also been seen in association with other papulosquamous and acantholytic disorders. A 50-year-old woman with an 8-year history of generalized pustular psoriasis (GPP) presented for multiple vesicles and punched out crusted ulcers on her face and neck, which had appeared initially 3 days ago. She had been treated with cyclosporine 200mg/day from 1 week ago because of the exacerbation of GPP. A diagnosis of KVE was considered, and polymerase chain reaction examination for HSV was positive. Although KVE in patients with psoriasis is not common, dermatologists need to pay attention to the risk of psoriasis herpeticum during the use of immunosuppressants.
