Purpose: In the present age of information technology, apart from software size, the proportion of software in systems has increased in most of industries. Hence, software-related system failures have attracted the attention of engineers. The objective of this study was to develop a failure mode and effect analysis (FMEA) procedure for identifying critical failure modes of software.
Methods: We modified the software FMEA procedure of Neufelder [1] by deleting unnecessary steps and introducing new methods for assessing the severity and likelihood of occurrence and detection of failure modes of software. We applied the developed FMEA procedure to the user authentication module of a public service system that is part of the e-government system.
Result: We propose a software FMEA procedure that can be used for developing or modifying software applications. The proposed procedure is effective in identifying critical failure modes of software.
Conclusion: We propose a new software FMEA procedure that can be used for managing the reliability of software under development and applied it to the user authentication module of a public service system that is part of the e-government system.
