전 세계적으로 암 사망자 수 1위인 폐암에 대해 저선량흉부 컴퓨터단층촬영을 통한 조기진단의 효과가 국외 여러비교임상 연구들을 통해 제시되면서 고위험군을 대상으로한 폐암 검진이 권고되고 있다. 국내에서도 국립암센터 중심으로 근거 기반 연구를 통한 폐암 검진 권고안이 2015년에 제시되었고, 전국 단위 시범사업을 거쳐 2019년부터 국가 폐암 검진 사업이 시작되었다. 국내 시범사업에서 폐암검진의 효과를 확인한 미국 연구보다 강화된 양성 판정 기준을 적용하고, 정보시스템을 이용한 검진 질관리, 검진 후결과 상담 및 의무적인 금연 상담 제공을 통해 국외 임상연구들과 비교하여 유사하게 높은 폐암 조기발견율을 얻었지만, 위양성률과 확진검사 과정에서의 부작용은 감소시킬수 있었고, 수검자의 심리적 불안이 높지 않았던 반면, 금연의지는 높일 수 있었다. 본고에서는 국내 폐암 검진 사업 도입의 근거와 시범사업을 통해 국내 인구 기반으로 확인된폐암 검진의 효과와 문제점을 살펴보고, 국가 폐암 검진 사업 도입 이후 보완해 나가야 할 과제를 제시하였다.
