Objective: The purpose of this study is to identify and organize team decision making (TDM) performance factors and apply them to the investigation of previous accidents in complex systems.

Background: Team work, particularly TDM, is increasingly important to the safe and efficient operation of complex socio-technical systems such as nuclear power plants (NPPs). For this reason, identification and organization of TDM performance factors has recently been an important issue.

Method: This study established a model for identifying and organizing TDM influencing factors and then identified 34 TDM influencing factors based on the model through a comprehensive literature review and accident analysis. In order to understand how the 34 factors can be reasonably used as causal factors of accidents and to examine how each of them significantly affects TDM process, we examined 558 accidents in three domains (NPPs, aviation, and railway).

Results: The 34 factors were useful for investigating accidents in which TDM process was critical and they have different significance in affecting the performance of TDM.

Conclusion: The proposed model and the 34 factors would be useful for examining accidents in which TDM performance is problematic.

Application: The proposed model and the 34 factors would be effectively used for modeling and analyzing accidents in complex systems and would be a theoretical basis for probabilistic safety assessment (PSA) in severe accident situations.
