충수염은 전형적으로 배꼽 부위에서 우하복부로 옮겨지는 복통, 발열, 오심, 구토 등의 증상을 보일 수 있어 자세한문진과 신체 검사만으로도 진단이 가능하다. 그러나 비특이적인 증상을 호소하는 경우가 있으며, 충수의 위치나 충수염의 양상에 따라 다양하게 나타난다. 충수염으로 인한 천공이생겼을 경우 전신상태가 급격히 악화되어 응급수술이 필요한 경우가 흔하나, 국소적인 충수주위농양을 형성하며 뚜렷한 증상을 일으키지 않는 경우도 있다[1]. 모호한 복통으로내원한 환자에서 염증성 장질환, 과민성 장증후군, 대장암등을 감별하기 위한 검사로 대장내시경을 시행하다 우연히 충수염이 진단되기도 한다. 이러한 경우 관찰되는 대장내시경 소견은 충수돌기 개구부의 융기, 발적과 부종, 화농성 삼출물 등이 대표적이다[2]. 저자들은 복부 불편감 없이 단순검진 목적으로 시행한 대장내시경 검사에서 충수돌기 개구부는 정상이며 구불결장에 융기된 상피하 병변이 관찰되어추가로 시행한 복부 전산화단층 촬영에서 충수염과 충수주위농양을 진단하고 치료한 증례를 경험하였기에 문헌고찰과함께 보고하는 바이다.
