Ptosis has multiple etiologies, including supranuclear lesion, lesion of the oculomotor complex, oculosympathetic lesion, lesion of the neuromuscular junctions, disease of the muscle and local mechanical lid abnormalities. Unilateral isolated supranuclear ptosis is known to be caused by cortical lesion, a condition called “cortical ptosis”. Thalamic infarction may cause hemiparesis, hemiataxia, memory deficit, oculomotor abnormality and eye lid abnormality. However isolated ptosis without miosis and anhidrosis in the thalamic infarction is not reported. We describe a patient with ipsilateral isolated ptosis with thalamic infarction. A 55-years-old right handed man admitted due to right ptosis without any other neurologic deficits. Brain magnetic resonance imaging revealed single infarction in the right antero-lateral thalamus. We postulate that the thalamic infarction may induce ipsilateral isolated ptosis like to cortical ptosis due to disruption of the thalamocoritcal connection.
