Ischemic stroke caused by spontaneous calcific emboli (SCE) is very rare and had rarely been known about responsiveness
to thrombolytic therapy. We present a case that was treated successfully by intravenous (IV) tissue plasminogen activator
(tPA) for acute ischemic stroke caused by SCE. A 85-year-old woman presented with left-sided weakness of sudden onset.
Emergent brain computerized tomography (CT) and CT angiography showed no definite acute ischemic changes except an
old left occipital infarct and high-attenuation lesions containing calcium in the proximal A3 segment of the right anterior
cerebral artery (ACA) and P1 segment of the left posterior cerebral artery. She was treated with IV tPA 1.5 hour after
symptom onset. She had made an almost complete recovery of motor weakness during infusion of tPA, and showed scattered
acute ischemic lesions in the right ACA territory on magnetic resonance imaging (MRI) performed in the following day.
