Percutaneous gastrostomy is an excellent alternative to parenteral and nasogastric feeding for patients unable to maintain adequate oral nutrition. Gastrostomy tubes can be placed surgically, endoscopically, and radiologically. Percutaneous endoscopic gastrostomy (PEG) continues to be more frequently performed than other methods. A 77-year-old man has kept the nasogastic tube for 3 months due to swallowing difficulty caused by cerebral infarction. Through the endoscope we could find that his stomach was positioned abnormally high. To avoid the potential injury on the other organs, we didn’t perform PEG. Con-sidering age and cardiovascular disease, surgical operations were also poor option, thus we tried percutaneous radiologic ga-strostomy (PRG). Unlike the PEG which punctures vertically along the light source by endoscope, PRG uses a fluoroscope and it is possible to insert an angled needle with the tube in left upper abdomen. In conclusion, PRG is recommended for the patients who fail for PEG. (Korean J Stroke 2008;10:45-47)
