The purpose of this study is to examine how aqua therapy and treadmill walking exercise for 8 weeks influ-enced daily physical fitness and inflammatory markers in stroke hemiplegia patients. The subjects were select-ed if they diagnosed as hemiplegia caused by a stroke, could walk independently using at least walking aids, and had no lower motor neuron lesions or orthopedic diseases of lower extremities and randomly assigned into the aqua therapy group(n=9) and the treadmill walking group(n=10). Measurement was taken in body composition(weight and %body fat), daily physical fitness(grip strength of the unaffected side, standing up and sitting down a chair, sitting trunk flexion, one-leg-standing of the unaffected side with eyes open, bass test of dynamic balance, and 6-minute walking), and inflammatory markers(CPR, IL-6, and TNF-α). After exercise for 8 weeks, the results were obtained as follows. First, for the changes in daily physical fitness, the aqua therapy group had positive effects on standing up and sitting down a chair, sitting trunk flexion, and bass test of dynamic balance, whereas there was no significant difference in grip strength of the unaffected side, one-leg-standing of the unaffected side with eyes open, and 6-minute walking. The treadmill walking group had positive effects on grip strength of the unaffected side, sitting down a chair, sitting trunk flexion, one-leg-standing of the unaffected side with eyes open, bass test of dynamic balance, and 6-minute walking. Second, for the changes in inflammatory markers, the aqua therapy group had positive effects on CRP, whereas there was no significant difference in IL-6 and TNF-α. The treadmill walking group had positive effects on CRP and TNF-α, whereas there was no significant dif-ference in IL-6. Taken together, regular exercise influenced positively the improvement of daily physical fitness and inflammatory markers which are known for the risk factor of cardiovascular diseases in stroke hemiplegia patients. Further researches should develop a variety of exercise programs and suggest exercise to improve the functions of the affected side in hemiplegia patients.
