Objectives Nowadays people usually perceive Doyin(導引) as a means of preventing disease and improving health. However from the past, Doyin has been used for the purpose of treating disease. Especially Five-viscera-doyinbub(五臟導引法) in 『Donguibogam(東醫寶鑑)』 is more obvious for its therapeutic purpose. This study focused particularly on Spleen-doyinbub(脾臟導引法) among Five-viscera-doyinbub. The reason is there are many patients with digestive diseases today and chronic digestive problems often degrade the quality of life. In Korean medicine, digestive diseases are recognized as a category of Spleen-stomach-diseases(脾胃病). But there is a very lack of research that uses Doyin to treat Spleen-stomach-diseases. Therefore the objective of this study is to consider the movements and effects of Spleen-doyinbub and to arrange diseases that can be clinically applied. Futhermore, I want to propose a method of using Spleen-doyinbub to treat digestive diseases in clinical practice.
Methods First, in this study Doyin and Doyin in 『Donguibogam』 were examined through precedent research and related texts, then I considered the clinical meaning of Doyin. Second, the movements and effects of Spleen-doyinbub were considered through related papers and books. In addition, diseases from 『Donguibogam』 that equivalent to the Spleen-stomach-diseases were selected as an indication of Spleen-doyinbub. The indications were divided into three categories: ‘Internal damage and Spleen-stomach-diseases(內傷脾胃病)’, ‘External body elements(外形) and six-Qi(六氣)’, and ‘Miscellaneous disorder(雜病) accompanied by Spleen-stomach-disease’. Also incurable diseases that are difficult to move the limbs are excluded. In this study, the scope of application of Spleen-doyinbub was limited to Spleen-stomach-diseases in order to apply it for clinical purposes. Accordingly, it is revealed that the study was conducted only on diseases caused by problems with stomach, earth-Qi(土氣) and eating habits.
Results and conclusions Doyin refers to a way to control the breathing and move the body, ultimately training the body and mind together. The effect of Doyin goes beyond recovering motor skills through body movements, promoting circulation of Qi(氣) and blood(血) inside the body, facilitating the operation of the meridians through muscle stimulation, and stabilizing the mind. Doyin also has the meaning of guiding people to conform to the laws of ‘people of Sang-go(上古之人)’ in the 『Huangdineijing(黃帝內經)』, so it can be a way to lead them to the complete health pursued by Korean medicine. Therefore in clinical practice, areas of using Doyin can cover both prevention, treatment, and care. Ultimately, Doyin becomes a means to lead people to achieve high quality of life on their own.
The significance of Doyin in clinical practice is first to treat diseases and to control the body, and secondly, to control the mind by keeping spirit(神) intact. In particular, the Five-viscera-doyinbub in 『Donguibogam』 has a clear therapeutic purpose compared to other Doyinbub as the efficacy is concentrated in the five-viscera(五臟). Therefore in clinical practice, the medical significance of Doyin can be realized by prescribing appropriate Five-viscera-doyinbub according to the appropriate five viscera diseases in clinical practice.
The first movement of Spleen-doyinbub in 『Donguibogam』 is to sit back, bend one leg and fold the other leg, with both hands facing back, touch the ground and pull the body upside down. This movement has the effect of moving the body along the distribution range of spleen meridian(足太陰脾經) and foot Tae-eum muscle meridian(足太陰經筋). It also stimulates the hip joint, preventing the pathogenic-Qi(邪氣) from staying in the eight- deficiencies(八虛) area of the spleen. And it also helps the essence-of- spleen(膏) enter Eumgo(陰股). The second movement of Spleen-doyinbub is to look back as hard as a tiger looks at it, while kneeling down and leaning on the ground with hands (or taking the body in the form of a magic square on the ground). This action also has the effect of stimulating around the BL20(脾兪穴).
For the clinical use of Spleen-doyinbub, I selected diseases in 『Donguibogam』 that are directly related to spleen and stomach as indications of Spleen-doyinbub. About the indications, there are three categories: ‘Internal damage and spleen-stomach-diseases(內傷脾胃病)’, ‘External body elements(外形) and six-Qi(六氣)’, and ‘Miscellaneous disorder(雜病) accompanied by Spleen-stomach-disease’. In organizing the indications by 『Donguibogam』, ‘Internal damage and spleen-stomach- diseases’ based on 「Spleen(脾臟門)」, 「Stomach(胃腑門)」, 「Internal damage(內傷門)」, ‘External body elements(外形) and six Qi(六氣)’ based on 「Mouth and Tongue(口舌門)」, 「Abdomen(腹門)」, 「Flesh(肉門)」, 「Dampness(濕門)」, and the ‘Miscellaneous disorder(雜病) accompanied by Spleen-stomach-disease’ based on 「Deficiency Vexation(虛勞門)」, 「Intestinal Convulsion(?亂門)」, 「Vomiting(嘔吐門)」, 「Accumulations(積聚門)」, 「Edema(浮腫門)」, 「Distention(脹滿門)」, 「Wasting-thirst(消渴門)」 and 「Jaundic(黃疸門)」.
In Korean medicine, usually certain symptoms do not determine a particular disease. Even the same diseases may have different symptoms depending on the causes, or sometimes the symptoms are similar even though the causes are different. Therefore, the effective utilization of Spleen-doyinbub requires the ability to diagnose that the patient''s symptoms are due to spleen problems, beyond observing individual symptoms suspected of a Spleen-stomach-disease. For this, the process of diagnosis with the diseases specified in the Korean medical classics should be preceded rather than based on a single symptom. For the effective use of Spleen-doyinbub, the name of the disease based on the Korean medical classics should be recorded in the medical records. This is because Korean medicine disease names are based on the physiological and pathological concepts of Korean medicine, so the cause, mechanism, and purpose of treatment are different compared to Western disease names. Therefore, it is difficult to completely replace the Korean medicine disease names with that of the West. Thus, it is necessary to improve the medical system so that more accurate records can be made when Korean medicine doctors use Doyin in clinical practice.
In conclusion, Spleen-doyinbub in 『Donguibogam』 is worth applying as a treatment method to patients with Spleen-stomach-diseases in clinical practice. And additional clinical research and institutional supplementation are required for effective application.
