치과의료기관 유형에 따른 감염관리체계와 이행실태를 파악하고자 2020년 4월 1일부터 5월 10일까지 전국을 대상으로 지역과 기관유형을 분류하여 대학·종합병원 치과 35개소, 치과병원 41개소, 치과의원 121개소를 층화무작위표본추출법으로 모집하였다. 기관유형별 감염관리 체계, 진료실 환경 및 시설관리, 인력 안전관리, 기자재 멸균관리, 의료폐기물 및 세탁물 관리 실태를 구성하는 각 항목은 Chi-square test와 Fisher’s exact test로 분석하였고, 영역별 항목을 합산하여 100점으로 환산한 후 one way analysis of variance를 통해 다음과 같은 결론을 얻었다.
안전한 의료서비스를 제공하기 위해 감염관리수준을 전반적으로 높여야 하므로 미흡한 영역에 대한 자각과 개선의 노력이 요구되었으며, 특히 치과의원의 관리가 시급하였다. 이를 위해 의료기관 자체의 감염관리체계 구축과 실천에 대한 노력이 우선되어야 하지만 다양한 정보를 제공하고 지원하며, 감시를 병행하는 제도적 인프라 구축이 필요할 것으로 사료된다.
From April 1, 2020 to May 10, 2020, dental institutions were classified into regions and institutions and 35 university hospitals, general hospitals, 41 dental hospitals, 121 dental clinics were recruited through a stratified random sampling method to investigate status of infection control by type. Each item constituting the infection control system by dental institution type, treatment room environment and facility management, personnel safety management, sterilization management of equipment, dental waste and laundry management were analyzed as Chi-square test and Fisher’s exact test. After summing up each items and converting them into 100 points for each area, one way analysis of variance was conducted to obtain the following conclusions.
The level of infection control system establishment and operation by type of dental institution was 89.0 points for university and general hospitals, 75.6 points for dental hospitals, and 34.4 points for dental clinics(p<.001).
In the environment and facilities of treatment room, air(p<.001), dental unit waterline(p<.001), surface(p<.001) and operating room management levels were 76.1 points, 72.5 points, 73.0 points, and 74.0 points for university and general hospitals, similar to 77.2 points, 75.1 points, 71.0 points, and 73.8 points for dental hospitals, while 61.1 points, 40.0, 37.0 points and 45.6 points for dental clinics. Prevention and management of staff infection exposure(p<.001), wearing personal protective equipment(p<.05), hand hygiene(p<.001) levels were 90.7 points, 75.5 points and 88.5 points for university and general hospitals, 79.8 points, 79.5 points, and 80.4 points for dental hospitals, and 50.2 points, 88.0 points and 61.5 points for dental clinics. The sterilization management system was 90.8 points for university and general hospitals, 79.5 points for dental hospitals, and 64.6 points for dental clinics(p<.001). The institutions that abide by the rules of a high-risk, semi-critical and medical disposable devices management were 69.7%, 63.4%, 51.4% of university and general hospitals, 66.7%, 68.7%, 41.0% of dental clinics, and 48.7%, 41.9%, 27.7% of dental hospitals. The majority of dental institutions had autoclaves but the level of retention by type and the sterilization monitoring had many differences(p<.001). The dental institutions using all three methods were 71.4% of university and general hospitals, 48.8% of dental hospitals, and 11.6% of dental clinics(p<.001). Medical waste management(p<.01) and laundry management(p<.001) had 92.0 points and 83.4 points for university and general hospitals, 82.9 points and 74.1 points for dental hospitals, while 77.1 points and 40.3 points for dental clinics.
Efforts were required to become aware of and improve areas of insufficient infection control in order to provide safe medical services and Infection control level should be raised overall, especially the management of dental clinics were urgently needed. To this end, efforts to establish and practice the infection control system of the dental medical institution itself should be prioritized but it is considered that institutional infrastructure to provide and manage various information and monitor the system is needed.
