우울증은 장·단기적으로 일상생활에서 쉽게 경험할 수 있는 정신질환으로, 인구사회학적, 건강행태 요인과 관련이 있으며, 생활습관병, 불규칙한 식습관이 우울에 영향을 미친다. 또한 영양소는 우울증의 발병과 지속기간에 중요한 역할을 하는 것으로 알려져 있다. 하지만 우울증의 예방과 그에 따른 관리 대책 마련이 시급한 실정임에도 불구하고 현실은 우울증을 초기에 판별하기가 어려워 조기에 발견하는 것이 쉽지 않다. 우울증의 임상적인 진단은 이미 그 증상이 심화되고 있거나 심각할 것으로 판단하여 본 연구에서는 제7기 1차년도(2016) 국민건강영양조사 자료 내에 우울증상선별도구인 PHQ-9를 이용하여 점수를 두 집단으로 나누어 대상자로 하였으며, 일반적 특성, 건강행태, 건강상태, 영양소섭취의 차이를 비교하고, 건강행태, 건강상태, 식생활과의 관련성을 알아보고, 그 결과를 통해 우울증을 조기에 발견하여 그에 따른 올바른 식생활 및 영양소섭취를 관리할 수 있는 방향을 제시하기 위해 수행하였다. 자료는 SPSS 24.0을 이용하여 복합표본 χ2-test, 복합표본 로지스틱 회귀분석, 복합표본 t-test로 분석하였다.
일반적인 특성은 성별, 연령, 개인 및 가구소득수준, 교육수준, 기초생활수급 경험 유무, 주택 소유 유무, 결혼여부, 결혼상태에서 두 집단 간에 유의한 차이가 있었다. 건강행태는 주관적인 체형인식, 1년간 체중변화 여부, 1년간 체중 증가량, 체중조절 노력 여부에서 두 집단 간에 유의한 차이가 있었다. 우울증상의 수준에 영향을 미치는 요인을 분석하기 위해 복합표본 로지스틱 회귀분석으로 분석한 결과, 1년간 음주빈도, 한 번에 마시는 음주량, 폭음 빈도, 1년간 음주문제 상담 여부, 1년간 타인의 음주로 인한 피해 여부에서 높은 우울증상, 평소 스트레스 인지 정도, 1년간 자살 계획 여부, 1년간 자살 시도 여부, 1년간 정신문제 상담, 스트레스 인지율, 평생 흡연 여부, 니코틴 의존, 가정실내 간접흡연 노출 여부, 현재흡연율, 1주일간 걷기 일수, 1주일간 근력운동 일수, 유산소운동 실천율에서 높은 수준의 우울증상과 유의한 관련성이 있는 것으로 나타났다. 건강상태는 이완기 혈압, 혈당, 혈중 중성지방에서 두 집단 간에 유의한 차이가 있었다. 우울증상의 수준에 영향을 미치는 요인을 분석하기 위해 복합표본 로지스틱 회귀분석으로 분석한 결과, 주관적 건강상태, 우울증 의사진단 여부, 우울증 현재 유병 여부, 우울증 치료 여부, 최근 1달간 및 1년간 와병 여부, 고혈압 유병 여부, 당뇨병 유병 여부, 중성지방혈증 유병 여부에서 높은 수준의 우울증상과 유의한 관련성이 있는 것으로 나타났다. 영양조사는 식생활형편에서 두 집단 간 유의한 차이가 있었다. 우울증상의 수준에 영향을 미치는 요인을 분석하기 위해 복합표본 로지스틱 회귀분석으로 분석한 결과, 최근 1년 1주동안 아침·점심·저녁식사 빈도, 외식 횟수, 최근 1년 동안 아침·점심·저녁식사 시 가족 및 가족 외 사람과의 동반 여부에서 높은 수준의 우울증상과 유의한 관련성이 있었다. 에너지 및 영양소섭취는 19-29세의 여성을 제외한 연령층에서 대부분의 영양소가 고우울증상군이 저우울증상군보다 섭취량이 낮았다.
우울증과 관련된 여러 요인에 대한 관심이 증가하고 있는 추세에서, 본 연구는 혈액성상, 생활습관병 유병 여부, 영양소섭취와의 관련성을 분석한 것에 의미가 있다고 보며, 우울수준에 따른 전반적인 특성을 알 수 있었고, 우울증의 조기예방 및 관리의 방향을 제시하기 위해 영양학적 평가 및 중재가 필요하다고 사료된다.
