The purpose of this study is to investigate incidence rates of complications in response to the introduction of quality evaluation of the DRG(diagnosis related group) payments, focusing on an increasing number of patients with lens surgery as the population aging increases.
The results of the study are as follows. First, among lens surgery(cataract surgery) complications, there were 86 patients for vitreous prolapse, 87 patients for intraocular pressure(IOP) elevation, and 67 patients for other complications. By gender, men had higher distributions(62.8% of vitreous prolapse and 62.1% of IOP elevation) compared to those for women(37.2% of vitreous prolapse and 37.9% of IOP elevation), except for other complications(men with 49.3% and women with 50.7%). There was no statistically significant difference between male and female patients.
Secondly, lens surgery complications(cataract surgery) were the highest when the number of hospitalized days was one day, with 96.6% of IOP elevation, 94.2% of vitreous prolapse, and 73.1% of other complications. This was statistically significant(p<0.001).
Thirdly, in terms of the characteristics of lens surgery(cataract surgery) complications from different types of medical institutions, primary hospitals revealed the highest in all complications, including 79.1% of vitreous prolpase, 86.2% of IOP elevation, and 52.2% of other complications, and this was a statistically significant difference(p<0.001).
Fourthly, there were three dependent variables (''vitreous prolapse'', ''IOP elevations'', and ''other complications'') in this study, and therefore multivariate logistic regression was performed. The result of the analysis indicates that as the number of hospitalized days increased, vitreous prolapse decreased by 0.27 times(95% CI 0.08~1.00) and IOP elevation decreased by 0.14 times(95% CI 0.03~1.59), compared to other complications, and this was statistically significant.
From the above results, this study is meaningful in that it has compared the evaluation results of the appropriateness of DRG payment and the medical quality for lens surgery complications patients, in response to the introduction of quality evaluation by DRG payment.
