Introduction: The aim of this study was to analyze the SP/AP ratio value of electrocochleography(ECoG) according to the position of summating potential(SP) peak. We compared SP/AP ratio value between negative polarity graph and positive polarity graph of same ECoG in each patient. In addition, we attempted to evaluate the utility of 2 different manners of recording ECoG graph in diagnosis of Meniere’s disease.
Materials and Methods: Retrospectively, we analyzed the result of ECoG in sixty-seven patients with unilateral definite Meniere’s disease. ECoG was analyzed in two different manners. From action potential(AP) peak, the SP peak was determined to be close when positioned in negative polarity; on the other hand, SP peak was considered distant when positioned in positive polarity. The SP/AP ratio was interpreted with reference to a base line value. The ratio of two different ECoG value for each Meniere’s disease patient was calculated.
Results: For pathologic side of Meniere’s disease, negative polarity ECoG showed significantly greater value of SP/AP ratio (mean: 0.334 ± 0.10) than positive polarity ECoG (mean: 0.283 ± 0.09) (p=0.001). For normal side, negative polarity ECoG showed significantly greater value of SP/AP ratio (mean: 0.250 ± 0.09) than positive polarity ECoG (mean: 0.204 ± 0.06) as well as in pathologic side(p=0.001).
Conclusion: The value of SP/AP ratio was significantly greater when analyzed with negative polarity than positive polarity manner, in both pathologic side and normal side. The standard SP/AP ratio for diagnosis of Meniere’s disease could be variable according to the manner of determining SP peak.
