Purpose : The purposes of this study were to examine the factors that cause moral distress and to explore the ethical climate and moral distress in nurses who work in neonatal intensive care units.
Method : The participants in this study were 152 neonatal intensive care unit nurses at seven university hospitals. The data were collected from 22nd October to 22nd November 2013, using a questionnaire consisting of questions about the nurses'' perceptions of the ethical climate and the level of moral distress. The collected data were analyzed using the following metrics: percentages, mean, standard deviation, t-test, ANOVA, the Scheffe test, Pearson''s correlation coefficient, and by stepwise multiple regression using the PASW Statistics program version 20.0.
Results : The average perception with regard to the ethical climate score was 86.82 (9.15), and the average moral distress score was 108.37 (52.72). Regarding the perception of moral distress, statistically significant differences were found for marital status, clinical experience, and NICU service career. Perceptions of ethical climate and moral distress (r=-0.541, p<.001) were negatively correlated. Factors Influencing the perception of moral distress explained 45.1% of the variance in clinical experience, the NICU service career and the ethical climate (manager, doctor, and patient).
Conclusion: An organizational approach that considers the peculiarities of neonatal nursing is useful for understanding the moral distress of nurses in neonatal intensive care units. The development of effective interventions considering factors that cause moral distress in neonatal intensive care unit nurses as identified in this study is recommended.
