본 연구는 가슴압박소생술과 기본심폐소생술의 교육효과를 비교 검증하기 위하여 시행된 비동등성 대조군 사전사후 반복측정설계의 유사실험연구로 H시 소재 일개 초등학교 5학년 전체를 대상으로 실시하였다. 연구 참여자는 연구에 대한 참여 동의서를 제출한 최종 연구대상자 68명이며, 자료수집 기간은 2013년 12월 2일부터 2014년 2월 7까지였다. 연구도구 중 수행자신감은 박정미(2006)의 도구를 일부 수정·보완하였으며, 가슴압박의 질은 Laerdal사의 Resusci Anne W/SkillReporter 평가형 마네킹으로 가슴압박 깊이(mm), 가슴압박 속도(회/분), 분당 압박 횟수(회), 압박위치 불량(회), 가슴압박 정확도(%)에 대한 5가지 항목을 교육 직후와 교육 8주 후 2회에 걸쳐 평가하였다. 자료분석은 IBM SPSS Statistics 21 프로그램을 사용하여 반복측정분산분석, 독립t-검정, 대응t-검정을 사용하였으며, 가슴압박소생술군을 실험군으로 기본심폐소생술군은 대조군으로 하였다.
주요 연구결과는 다음과 같다.
1. 실험군과 대조군의 수행자신감은 집단 간 유의한 차이가 없었으나, 시간 경과에 따라 유의한 차이를 보여 교육 직후에 가장 높았으며(p<.001), 사전검사에서 가장 낮았고(p=.001), 교육 8주 후는 사전검사보다 높았지만 교육 직후보다는 낮게 나타났다(p<.001).
2. 실험군과 대조군의 분당 가슴압박 횟수는 교육 직후 실험군이 130.0±9.38(회), 대조군은 95.1±11.82(회)로 실험군에서 유의하게 높았으며(p<.001), 교육 8주 후에는 실험군이 124.0±14.89(회), 대조군은 90.8±14.89(회)로 실험군에서 유의하게 높았다(p<.001).
3. 실험군과 대조군의 시간 경과에 따른 차이를 보면 압박 속도는 대조군에서 통계적으로 유의한 차이로 저하되었으며(t=-2.400, p=.022), 분당 압박 횟수(t=-2.078, p=.046)와 가슴압박 정확도(t=-2.248, p=.031)는 실험군에서 통계적으로 유의한 차이로 떨어져 교육 8주 후 지속성은 유지되지 않았다.
결론적으로 심폐소생술에 대한 수행자신감은 시간 경과에 따라 유의하게 낮아져 지속적인 심폐소생술의 반복교육이 필요하다. 질적인 측면에서 가슴압박소생술은 분당 압박 횟수에서 유의하게 높았으나 과다한 압박 속도로 인하여 적정 압박 횟수를 보이지 못했다. 그러나 압박 깊이, 압박 위치, 압박 정확도에서는 기본심폐소생술보다 높은 점수를 보여주어 인공호흡을 어려워하는 초등학생에게 가슴압박 수행능력을 높이는 방법으로 적용해 볼 수 있다. 또한 교육 8주 후의 지속성은 유지되지 않아 술기 중심의 반복교육이 필요하며, 가슴압박 정확도를 높일 수 있는 방안이 필요하다.
