Transcranial magnetic stimulation (TMS) is a noninvasive method of cerebral cortex stimulation. TMS has been used to treat neurological, psychological, and chronic neuropathic pain. The effects of TMS in the treatment of chronic spinal pain including back and/or leg pain have not yet been addressed. In the present retrospective study, we evaluated the effects of TMS on the clinical outcomes of 126 patients with chronic spinal pain, depression and insomnia secondary to pain. Clinical outcomes were assessed using a questionnaire featuring a numerical rating scale (NRS) for pain; the Beck depression inventory (BDI) to measure depressive mood; the insomnia severity index (ISI) to evaluate sleep problems; and the pain disability index (PDI) to assess restriction of daily activities. All measures improved significantly, as compared to pretreatment values, 1 week after the last of 15 sessions of TMS therapy (P < 0.001). No serious complications were noted although five subjects complained of mild transient headache. TMS therapy is thus both effective and safe and can be viewed as an add-on treatment modality for patients with chronic spinal pain and associated depression and insomnia.
