본 연구의 목적은 sacro occipital technique (SOT)을 이용한 카이로프랙틱이 만성요통환자의 통증과 신체기능에 미치는 영향을 규명하는 것이다. 만성요통이 있는 여성 45명을 세 실험군에 15명씩 무선할당하여 diversified technique (DT), SOT 또는 두 테크닉을 혼합하여 주 2회, 8주간 척추변위인 서블럭세이션과 픽세이션을 찾아 시술하였다. 무처치 대조군은 요통이 없는 건강한 여성 12명
으로 구성되었다. 시술 전, 4주후, 8주 후에 요통은 시각적상사척도 및 Oswestry와 Roland-Morris 요통장애지수로 측정하였고, 신체기능은 요부 근력과 유연성 및 체형으로 평가하였다.
처치 4주와 8주후 대조군에서는 유의한 변화가 나타나지 않은 반면, 세 실험군 모두에서 요통강도와 요통장애지수가 유의하게 감소하였으며(모두 p<.001), 세 실험군 간에 4주와 8주후 변화량의 유의한 차이는 보이지 않았다. 요부신전근력은 사전과 비교하여 4주와 8주 후에 유의하게 강화된 것으로 나타났으며(모두 p<.01), 세 실험군 간에 4주와 8주후 변화량의 유의한 차이는 보이지 않았다. 굴곡, 신전, 회전, 측굴 시 요부 유연성은 유의하게 향상되었으며(모두 p<.05), 4주와 8주후 변화량은 대조군과 비교해서 신전에서는 모두 유의하게 큰 것으로 나타났으며(모두 p<.05), 세 실험군 간에 4주와 8주후 변화량의 유의한 차이는 보이지 않았다. 어깨와 골반 균형은 8주후 세 실험군 모두에서 유의하게 향상되었으며, 좌우 다리길이의 차이는 혼합 처치군에서만 8주 후에 유의하게 감소하였다(p<.05). 신체의 좌우 대칭정도는 두 단일 처치군에서만 유의하게 향상된 것으로 나타났다(p<.01).
본 연구의 결과 척추교정을 통한 만성 요통환자의 통증감소와 신체기능 향상의 효과가 직접적인 척추교정을 하지 않은 SOT를 통해서도 나타났다. 또한 두 테크닉을 병행 처치한 결과 단일 테크닉으로 시술한 경우와 비교하여 부가적인 효과는 없는 것으로 나타났다. 따라서 만성 요통환자들에게 척추교정을 하지 않더라도 SOT를 통한 연부조직이완요법만으로도 임상에서 효과적이고 안전하게 사용할 수가 있으며, 척추교정을 두려워하는 대상자나 척추교정을 하기에 부적합한 노인이나 어린이에게 SOT는 하나의 대안이 될 수 있을 것으로 기대된다.
