-연구목적:본 연구는 SNOMED CT를 활용하여 주호소·주진단 Reference set를 구축함으로써, 주호소를 기반으로 주진단을 효과적으로 선정할 수 있는 방법을 제안한다.
-연구방법:서울대학교병원의 90개 진료과에서 1년간 사용된 96,860건의 주호소 용어를 진료과별로 분석하여, 환자의 증상을 바탕으로 다양한 주호소가 입력된 호흡기내과를 연구 대상 진료과로 선정하였다. 이후, 호흡기내과에서 지난 10년간 사용한 18,686건의 주호소 및 주진단 데이터를 수집하여 분석에 활용하였다. 데이터 전처리 과정에서는 SNOMED CT의 최신 업데이트를 반영하여 주호소 용어를 재매핑하고, SNOMED CT concept ID를 기준으로 주호소의 사용 빈도를 분석하였으며, SNOMED CT 매핑 가이드라인에서 제시한 chief complaint의 target semantic tag에 부합하는 ‘finding’에 해당하지 않는 Semantic tag 값은 제외하였다.
-연구결과:정제된 데이터를 사용하여 주호소와 주진단 간의 연관성을 통계적 및 시각적으로 분석하기 위해 데이터마이닝 기법인 연관 규칙을 활용하였다. 이 분석은 R 프로그램의 arules 패키지와 apriori 알고리즘을 사용하여 수행되었으며, 지지도, 신뢰도, 향상도를 기준으로 분석 후 향상도를 기준으로 총 21개의 주호소에 대한 주진단 Reference set를 성공적으로 구축하였다.
-결론:이 Reference set 세트는 주진단의 정확성을 향상시키는데 큰 잠재력을 보여주며, 임상의사의 진단 과정을 보다 효과적으로 지원하는데 기여할 것이다.
