Lymphangioma is an uncommon lesion occurred by malformation of the lymphatic system. It is commonly found in the head and neck region, especially at the tip of the tongue causing macroglossia. The difficulty in maintaining oral hygiene and dry mouth with openbite, caused by a large tongue, contribute to severe dental caries. Also, it may interfere with the treatment and the patient’s respiration, dental treatment in outpatient settings is not recommended for these patients. In this report, we present a 4-year-old girl with lymphangioma who received dental treatment under general anesthesia at Seoul Asan Medical Center. She had macroglossia and openbite with poor oral hygiene. Depending on the results of clinical and radiological examinations, most of her primary teeth had dental caries. We extracted all her primary anterior teeth, and stainless steel crowns were placed on all remaining primary molars. The treatment was conducted on an inpatient basis, and fortunately, she was discharged safely. For patients with lymphangioma, even with treatment under general anesthesia, there may still be difficulties in securing patients’ respiration after surgery. Therefore, prevention should be prioritized through the combined efforts of the patient and the pediatric dentist. If treatment is needed, collaboration with the other medical teams is necessary. And early macroglossia treatment may be helpful to prevent multiple dental caries treatments and difficult orthodontic treatment or surgery.
