Autism is a developmental disorder characterized by severe deficits in social interaction and communication. Children with autism exhibit a wide spectrum of medical and behavioral symptoms, which makes routine dental care very difficult for these patients. Attention-deficit/hyperactivity disorder (ADHD) is characterized by inattention, impulsivity, and hyperactivity. The more severe a patient’s symptoms of ADHD, the more their anxiety about dental treatment. In this report, we report the case of an 8-year-old boy with autism and ADHD who presented with multiple caries and was successfully treated under general anesthesia. Due to uncooperative behavior related to autism and ADHD, all procedures including preoperative blood tests, periapical radiography, and dental restorative treatment were performed under general anesthesia. Because the patient was resistant to enter the outpatient anesthesia room, oral midazolam was administered for preanesthetic sedation. After 20 minutes, the patient became drowsy and was transferred to the operating room. Oral premedication with midazolam was effective in reducing induced anxiety in highly uncooperative patients.
