This study shows workflow based development and implementation of failure reporting, analysis, and corrective action system (FRACAS) processes. We utilize business process management system (BPMS) to design and execute FRACAS process model including documents and failure data models. By using components of BPMS, FRACAS processes can be clearly defined as workflow processes which have their standard roles, activities, resources and cycle times. And it provides web based form generation and database connection for modification and customization. We develop a prototype FRACAS using an open-source BPMS, and the result represents that the proposed work enables dynamic failure reporting and analysis of FRACAS processes which support various failure modes of parts and items.
