이 기술적 고찰은 중증 급성 호흡기 증후군을 치료 중인 코로나 바이러스 2 (SARS-CoV-2) 환자에서 신경근 차단제(neuromuscular blocking agents, NMBA) 사용에 대한 의학적 증거를 평가한다. 이 시점에서 대규모의 전향적 무작위 대조 시험(RCT)이 부족하므로, NMBA의 적응증과 유용성을 확인하기 위해 소규모 관찰 연구와 사례군 연구가 검토된다. 여기에 덧붙여서, 유사한 임상 시나리오를 다루는 대규모 RCT를 검토한 내용을 COVID-19 환자의 임상상황으로 재해석한다. 특히, NMBA는 기관내 삽관 중에 환자의 기침 및 의료진의 감염 위험을 최소화하는 데 도움이 될 수 있다. NMBA는 또한 환자, 특히 심각한 임상 증상이(H 표현형) 있는 환자에서 기계적 환기에(MV) 필요한 구동 압력을 줄이면서 환자와 인공 호흡기 동기화를 촉진하는 데 사용할 수 있다. 복와위는 SARS-CoV-2 급성 호흡 곤란 증후군(ARDS) 환자의 불응성 저산소혈증 관리의 초석이 되었으며, NMBA는 이러한 조작을 용이하게 하는 데 유용할 수 있다. 수술 전후 설정에서 깊은 수준의 신경근 차단은 복강경 수술 중 환자의 결과를 개선할 수 있으며, 더 낮은 흡입 압력이 활용될 수 있으므로 이론적으로 에어로졸화의 위험을 줄일 수 있다. 적응증에 관계없이 정량적 신경근 감시는 신경근 차단의 중단 후 적절한 회복을 확인하는 방법 중 유일하게 신뢰할 수 있는 방법으로 남아 있다. COVID-19 환자에서 신경근 차단 감시의 적용은 특별한 이점을 제공하는데 말초 신경 자극기를 사용한 자동화된 측정을 함으로써, 주기적인 주관적 평가 중 발생하는 의료진과 환자의 접촉을 줄일 수 있기 때문이다.
