Background/Aims
A sixty nine year-old male admitted for surgical treatment of advanced gastric cancer diagnosed in a local medical clinic. He had underwent esophagogastrofiberscopy in the local clinic, and therefore a huge ulceroinfiltrating lesion was noted in the body of stomach. In the preoperative evaluation, distant metastasis was not found.
Methods
Five trochars were established similarly as conventional laparoscopic gastrectomy. First trochar was established by the umbilical incision, and a flexible scope was inserted through this port. In the laparoscopic view, serosal invasion was noted on the posterior wall of gastric body. In addition, this lesion abutted to the pancreas body, tail, and transverse mesocolon. Therefore, we planned the laparoscopic combined resection(1). To achieve a curative surgery, we performed total omentectomy and D2 lymph node dissection(2). In the process of lymph node dissection, the tumor-involved portions of pancreas and transverse colon were devascularized.
