Background and Purpose: Recently, it has been reported that the long-term outcome of patients with locally advanced pancreatic cancer (UR-LA pancreatic cancer) who underwent resection after chemotherapy (radiation) was excellent. However, not only the efficacy of chemotherapy but also the duration of treatment, timing of resection, and optimal regimen have not been sufficiently investigated. In this study, we evaluated the long-term results of resection after long-term chemotherapy for UR-LA pancreatic cancer and discussed the significance of the results. Patients and Methods: From January 2008 to December 2019, 76 patients diagnosed with UR-LA pancreatic cancer at the time of initial diagnosis were treated with chemotherapy for at least 6 months with the intention of tumor shrinkage, and resection was performed only in cases with a treatment effect of SD or better. Outcome: Seventy-six UR-LA pancreatic cancers were treated with chemotherapy (radiation), and 34 (44.7%) were resected.
