Breast cancer surgery in modern medicine was established by Dr. Halsted who started radical mastectomy. After Dr. Fisher and Dr. Veronesi have established breast conserving surgery, nipple sparing mastectomy (NSM) with reconstruction also has now been positioned as an option for breast cancer surgery. NSM has the advantage of improving aesthetic satisfaction as well as providing oncological treatment for breast cancer patient. However, conventional NSM has the disadvantage of leaving a significant scar on the breast. Robotic surgery has started to overcome this shortcoming.

Robotic mastectomy is robot assisted NSM. Conventional NSM needs a single incision of at least 6 cm in the breast, or two or more incisions even if the incision is smaller. With the robotic system, the operation can be performed with only a single incision of about 4 cm in the axilla area, so the location and small size of the scar make it inconspicuous.

Some people still misunderstand that robotic surgery is performed entirely by a robot without a surgeon, but it is the surgical modality that we surgeons are very familiar with. Although robotic systems have been used for over 10 years in many areas of surgical operation, robotic surgery in breast surgery has a very short history. Dr. Toesca reported the first robotic mastectomy in the world in 2016.
