Objective: To investigate the inter-rater and intra-rater reliability of the Korean version of the MACS by comparing the classification given by pediatric therapists and guardians of children with cerebral palsy.

Background: Cerebral palsy can affect hand functions resulting in diminished mobility, muscle weakness, insufficient control in rapid coordinated movements, involuntary movements, spasticity, and poor postural control. The Manual Ability Classification System (MACS) has been widely used to describe children"s hands with cerebral palsy when handling objects in their daily activities. However, the reliability of this MACS has not been verified in Korean.

Method: Data were collected from 45 children in the age range of 4~18 years old who were diagnosed with cerebral palsy by a medical specialist in rehabilitative medicine/neurosurgery. Total two pediatric therapists (physical therapist and occupational therapist) classified manual ability (MACS) through direct observation and information provided by guardians. Pediatric therapists and guardians used the Korean version of the MACS. Inter-rater and intra-rater reliability was analyzed using unweighted Kappa coefficient (k) and intra-class correlation coefficient (ICC).

Results: The results of intra-rater agreement show almost perfect agreement almost perfect between both pediatric therapists classified the MACS of children with cerebral palsy in the initial assessment and after one month. The results of inter-rater agreement between the physical therapist and the occupational therapist that MACS has almost perfect [K=0.87 (95% CI: 0.81~0.92), ICC=0.93 (95% CI: 0.91~0.97)]. The results of inter-rater agreement between physical therapist and guardians were observed to be moderate [K=0.45 (95% CI: 0.34~0.57), ICC=0.76 (95% CI: 0.64~0.84)].

Conclusion: Therefore, The Korean version of the MACS has been shown to be a reliable measurement tool that can be used jointly by pediatric therapists with variable clinical careers and by guardians.

Application: The application of the MACS in clinical practice can be an important focus to set treatment goals for disabled children including cerebral palsy in partnership with the guardians.
