Objective: The aim of this study is to propose a new approach to the categorization of human errors during the learning from the experiences of high reliability systems. Background: Human error is a revisit issue to cope with the new requirements of the increased social interest on the safety in ICT and high-reliability era. Human error studies have been limited by the traditional perspective to their classification and categorization, especially in retrospective stages. Method: A brief review on the human error classifications and categorizations will figure out the requirements for a new perspective on human errors in practice; (1) it should be more helpful to the extraction of countermeasures rather than the causes themselves to the human errors, (2) it should cover the new types of human errors such as ignorance, avoidance, forgery, violation, and concerns on cultural and organizational safety, (3) it should serve for obtaining the lessons learned from safety cases rather than causes. Results: A new approach to the investigation process including the classification is proposed based on the requirements on human error studies in practice. Conclusion: The proposed perspective and approach to human errors might help to enhance the efficiency and effectiveness of the studies to minimize the human errors and enhance the safety of high reliability systems. Application: The proposed approach to the classification and categorization of human errors could be incorporated into the various studies such as resilience engineering and big data as well as the investigation process on the safety/accident cases in practice.
