Type 1, II, III are regarded as "true" joint receptors, type IV is considered a classof pain receptor. Type 1, II and III mechanoreceptors, via static and dynamic input,signal joint position, intraarticular pressure changes, and the direction, amplitude,ancl velocity of joint movements. Type 1 mechanoreceptor subserve both static anclclynamic physiologic functions. Type 1 are founcl primarily in the stratum fibrosumof the joint capsule ancl ligaments. Type 1 receptors have a low threshold foractivation and are allow to aclapt to changes altering their firing frequency. Type IIreceptors have a low threshold for activation. These dynamic receptors respond tojoint movement. Type II receptors are thus termecl rapidly adapting. Type II jointreceptors are located at the junction of the synovial membrane and fibrosum of thejoint capsule and intraarticular and extraarticular fat pads. Type III receptors havebeen found in collateral ligaments of the joints of the extremities. Morphologicallysimilar to Golgi tendon organ. These dynamic receptors have a high threshold tostimulation and are slowly adating. Type IV receptors possess free nerve endingthat have been found in joint capsule and fat pads. They are not normally active,but respond to extreme mechanical deformation of the joint as well as to directchemical or mechanical irritation. Small amplitude oscillatory and distractionmovements(joint mobilization) techniques are used to stimulate themechanoreceptors that may inhibit the transmission of nociceptors stimuli at thespinal cord or brain stem levels.
