Background: The assessment of the severity of psoriasis isoften subjective because of the lack of quantitativelaboratory diagnostic tools. Histopathological examinationis the most commonly performed procedure for psoriasisdiagnosis; however, it is usually descriptive. Thus, there iscurrently no quantitative method of determining psoriasisseverity. The clinical types of psoriasis are correlated with theseverity of the disease, and a lesional severity index, such asthe psoriasis severity index (PSI), could be used as aquantitative tool for assessing gross severity. Objective: Tocorrelate the histopathological findings of psoriasis with thePSI. Methods: Psoriatic lesions in 98 patients were evaluated. The lesions were classified into the guttate, papular, smallplaque, and large plaque types according to morphology,and were scored according to the PSI. Ten commonhistopathological features of psoriasis were evaluated forcorrelation with gross severity. Results: The clinical types ofpsoriasis showed significant correlations with the histopathologicalseverity. However, the PSI score showed nocorrelation with histopathological severity. Conclusion: Inthe future, subjective gross assessment should be modifiedby using objective measuring devices with detailed scales, inorder to correlate the findings with the histological severity.
